Job Description

Family Continuity is seeking a full-time Care Coordinator/Benefits Specialist to serve as the Specialist for MassHealth insurance products and provides direct assistance to members in the Community Partnership (CP) program on accessing MassHealth benefits. The primary function of this position is to maintain continuity of care for CP members. This is also critical in supporting Care Coordinators to focus their time and attention on care coordination and treatment planning by removing this task from their workload and providing the time and attention to help members maintain or regain their healthcare benefits.

Essential Job Functions

  • Works directly with CP members to enroll in, maintain or regain MassHealth insurance, including telephonic and face-to-face contact in the office, community or members' residences.
  • Explain how the CP assists members in coordinating integrated healthcare and necessary services for members.
  • Consults with CP staff on MassHealth rules and protocols
  • Serves as liaison with MassHealth on behalf of CP members to ensure continuity of care in CP program
  • Document work with members and provide monthly reports of members served to CP and MassHealth
  • Consults with team and members about other public benefits, such as Social Security Income, Social Security Disability Income, and Medicare.
  • Perform administrative functions of the program, such as running reports, tracking PCTP's, etc.
  • Other duties as assigned.
